The Three Pathways to Eligibility
1. Means Test (Income-Based)
The most common route. The HSE assesses your net weekly income against a set of thresholds that vary by age, household composition, and distance from your GP. Many people calculate their gross income, assume they are over the limit, and stop there. That is a mistake.
2. Automatic Eligibility
Certain groups qualify without a means test. This includes people over 70 whose income falls within the relevant limits, children under 8, and recipients of certain DSP payments. If you or someone in your household falls into one of these categories, a means test is not required.
3. Discretionary Grounds (Hardship)
If your income is above the threshold but your medical expenses are significant enough to cause genuine financial hardship, the HSE may grant a card on discretionary grounds. This pathway is underused and often overlooked, particularly by families managing chronic illness or disability.
The "Allowable Expenses" That Most People Miss
